Payment & admissions

What it costs, and how someone moves in.

Rates depend on the level of care a person needs, which is why we quote after the assessment rather than before. Nothing about the process is designed to be fast — it is designed so nobody moves twice.

Paying for care

Three routes in.

Oregon Medicaid

We work with APD case managers and accept Medicaid placements. Your case manager handles the service plan and the rate; we handle the assessment and the room.

Private pay

Monthly rate is set by the care level established at assessment, with a written agreement covering room, board, care, and anything billed separately.

Long-term care insurance

We provide the documentation and care records most policies require. Bring the policy to the tour and we will tell you what it will ask of you.

The process

Four steps, in order.

STEP 01

Contact us

Call or send the form. Families and case managers both start the same way.

STEP 02

Come for a tour

See the home, meet our team, ask the hard questions. Bring whoever needs to see it.

STEP 03

Assessment

We review ADL needs, equipment, medications and medical history to confirm we can support them safely.

STEP 04

On-boarding

A written residency agreement and an individualized care plan, agreed before anything is signed.

STEP 05

Start service

A move-in date set around the family, with a care plan ready from the first day.

Bring these to the assessment

What we will ask for.

The clinical picture

Current medication list, recent hospital or facility discharge summary, physician contact, and any standing orders. If there is equipment, tell us the make and model.

The practical picture

Who holds power of attorney, who is the emergency contact, whether an APD case manager is involved, and what the funding route is likely to be.

The person

What their day used to look like. What they eat, when they sleep, what upsets them, what they enjoy. This is the part that makes the placement work.

Questions for us

Ask about staffing at night, what happens if their needs increase, how medications are handled, and what would make us say no. Good questions get straight answers.

Start with a phone call.

Families and case managers both start the same way. We respond the same day when we can, and always within one business day.
